Guardian (Legal Representative) Consent Form for the Accommodation of a Minor
Age Policy Summary
- Guests aged 19 or older, the age of majority under the law of the Republic of Korea, may book without any additional consent form.
- Minors aged 16 to 18 may stay without a guardian only if this guardian consent form has been completed and the original is presented at check-in, together with a copy of the guardian's identification document and the minor's own identification document.
- A minor who stays together with their guardian does not need to submit this consent form.
Guests under 16 years of age may not stay without an accompanying guardian, and this consent form does not permit such a stay.
If the original consent form is not presented, or if the information entered is untrue, check-in may be refused, and in that case the existing cancellation fee rules apply.
This consent form applies only when a minor aged 16 to 18 stays at Da-woo House without an accompanying guardian. The guardian (legal representative) must complete and sign this form in person, and the original must be presented at check-in together with a copy of the guardian's identification document and the minor's own identification document.

Terms of Consent
- ①I consent to the minor named above staying at Da-woo House alone, without an accompanying guardian.
- ②I confirm that the minor named above is 16 years of age or older, and I understand that a person under 16 years of age may not stay alone regardless of this consent form.
- ③I confirm that dormitory rooms are operated separately for men and women, and that the property complies with the prohibition of mixed-gender accommodation under the Juvenile Protection Act and other applicable laws.
- ④I acknowledge that the guardian is responsible for instructing the minor not to engage in conduct prohibited to minors by law, such as drinking alcohol or smoking, during the stay, and I agree that in the event of a violation the property may require immediate departure and that any refund shall follow the existing refund policy.
- ⑤If the minor damages the facilities or fixtures, or causes harm to other guests, the guardian shall be liable for compensation.
- ⑥I consent to the property contacting the guardian in an emergency, and I agree that if the guardian cannot be reached the property may hand the minor over to the police or another relevant authority.
- ⑦I agree to present the original of this consent form, a copy of the guardian's identification document, and the minor's identification document at check-in, and I agree that if these are not presented or the information entered is untrue the property may refuse check-in, in which case the cancellation fee rules apply.
- ⑧All civil and criminal liability arising from any false statement in this form rests with the guardian who completed it.
- ⑨I consent to the personal information in this form being collected and used solely for the purpose of confirming the stay and managing safety, and to it being retained for the period required by applicable law and then destroyed.
